So how do I sign-up?
You must be a depositor in the casino to be eligible.
Simply send us the following verification documents:
- Proof of Identification: An official document with a photo included such as a valid passport, driving license or permit, or an identification card.
- Proof of Address: If the above proof of identification does not verify your mailing address as listed in your casino account, you are required to send a recent utility bill or bank statement which will validate that address.
- Credit Card Verification: Copies of front and back of all credit cards you are using at the casino. If these are not available recent credit card statements should be provided.
- Confirmation Form: Signed & authorized deposit history for all previous credit card transactions. This will be requested in a separate e-mail once you ask to join the VIP Cashier Program.
You can photocopy and fax these documents to International fax: +1-613-482-5600. Alternatively you can scan or digitally photograph the documents, save them on your computer and e-mail them directly to support@clubhollywood.com.
Don't forget to include your name and casino username in the fax header or e-mail subject with the words "VIP CASHIER PROGRAM" clearly visible for quicker processing of your request. You will be contacted shortly by a representative with further instructions.
